Kerala Slips to Fourth in NITI Aayog’s Health Index

Kerala, long recognised for its strong public healthcare system, has been ranked fourth in the latest edition of NITI Aayog’s Sustainable Development Goal (SDG) 3 index on good health and wellbeing. This comes despite the state topping five out of the 11 health-related indicators in the report.

While Kerala had topped the SDG 3 rankings in the first two editions of the index in 2018 and 2019-20 with impressive scores of 92 and 82 respectively, it slipped in the rankings in subsequent years. A major reason for the drop was the inclusion of three new indicators in the 2020-21 edition: suicide rate, accident death rate, and out-of-pocket healthcare expenditure. That year, Kerala fell to the 12th position with a score of 72, below the national average of 74.

In the latest 2023-24 edition, Kerala has shown improvement by raising its score to 80, which is above the national average of 77. However, this was not enough to secure a place in the top three. Gujarat leads the rankings with a score of 90, followed by Maharashtra and Uttarakhand at 84, and Himachal Pradesh at 83. Karnataka shares the fourth position with Kerala.

Experts have pointed out that growing resistance to immunisation and institutional deliveries in certain parts of Kerala may have also impacted the state’s overall score.

Despite these setbacks in the health index, Kerala has continued to maintain its top position in the composite index of all 17 SDGs since the first edition, highlighting its overall strong performance across multiple development parameters.
